            Zahtevek mora biti vložen pri Ministrstvu za javno upravo, Tržaška cesta 21, SI-1000 Ljubljana, in sicer neposredno na tem naslovu, po pošti priporočeno ali priporočeno s povratnico. Vlagatelj revizije mora v zahtevku za revizijo navesti ime in naslov vlagatelja zahtevka za revizijo in kontaktno osebo, ime naročnika, oznako javnega naročila oziroma to odločitev, predmet javnega naročila, očitane kršitve, dejstva in dokaze, s katerimi se kršitve dokazujejo, pooblastilo za zastopanje v predrevizijskem in revizijskem postopku, če bo vlagatelj nastopal s pooblaščencem, ter priložiti potrdilo o vplačilu takse iz prvega in drugega odstavka 71. člena Zakona o pravnem varstvu v postopkih javnega naročanja (Uradni list RS, št. 43/11 in 60/11-ZTP-D, 63/13 in 90/14-ZDU-1I; ZPVPJN).
